It's teenage-ish slang, เค้า meaning "I" and usually paired with ตัวเอง meaning "you".

For meagoure:
The debate is around the 3rd word in your post "khao" which normally would mean him/her, but in this sentence (taking a hint from the slang-ish use of "pao") is meaning "I" (me). The difference in the Thai spelling is to do with the tone.ไอแอมฝาหรั่ง (iamguava)

i've heard เค้า being used to refer as "I" frequently but only by girls. is it true that guys dont use it. pls correct me if im wrong.

alvin you can use เอง like when you answer the phone and someone asks you who is it on the line then you can say นี่alvinเอง. if you are using it in terms of like "I or me" ตัวเอง would be more accurate.
