Khao is only used by girls. If any guys use it, he would be considered as "kathoey (labyboy)" or gay ka. However, some guys (must be very young) may use it with his gf, but it isn't normal case. It's better not to use for boys.

i did that in the past as well, even though I don't get to use it now, there are some circumstances depending on who you are talking to. For me, I use จ๋า/จ้ะ with most of the girls (friends, giเs, girlfriend). And เค้า/ตัวเอง , I use it with my girlfriend. I think it sounds lovely and cute.
