hi i opened a Ayutthaya bank account and got a visa debit card, i have a non immigrant o visa, 300bht for the card and 500bht minimum to put into your account, but this is your money so your only paying for the card, im not sure about yearly charges and i have not yet tried the online banking, but the card works abroad but online payments is not that easy as the card does not have your name on it, ive been told it takes one week if i want my name put on the card? its free to use the card in the area you take out the card, pattaya for me, but if i use in phuket or bangkok it will cost me 20bht per transaction even with the same bank, hope this helps someone, cheers

Pattaya is Chonburi province. I supposed they follow according to the province.

If you want more accessibility to ATM, local banks are better option.

This is the info regarding Kasikorn Bank's ATM withdrawal details. I suppose other banks' regulations like KTB and SCB are more or less similar.
